The Hazard Log is a configurable feature in Centrik 5. It helps you manage risks more effectively and it can be enabled for each Risk subsystem where you want to track hazards.
When activated, a new tab called Hazard Log will appear in that subsystem.
Recent improvements in the Risk module include better tagging and filtering features and this latest update support a new, flexible, tag-driven Hazard Log view. This view (tab) allows you to:
- Organise hazards using tags
- Filter hazards by tag
- Group hazards in ways that match your operational needs
The tag-driven approach is especially helpful in complex environments where many hazards need to be managed.

Viewing the Tag Hierarchy
Once tags have been assigned to hazards, you can view the tag hierarchy by selecting the Hazard Log tab. The hierarchy will be displayed, allowing you to explore the tags associated with each hazard.
Please note that hazards will show on the Hazard Log if:
- The hazard is directly linked to the tag.
- The entire risk assessment is linked to the tag, so all hazards within it are associated with the chosen tag(s).
Navigating the Tag Hierarchy
The tag hierarchy is designed to be intuitive and user-friendly. It includes:
- Expandable and collapsible sections to easily navigate through different levels of the hierarchy.
- Customizable filter options, so you can adjust the view based on specific criteria or preferences.

The ability to view aggregated scores at each level of the hierarchy for a comprehensive overview.
Viewing Hazards Associated with Tags
Each tag can be clicked to display a comprehensive list of all hazards associated with that particular tag. This feature allows users to easily filter and view hazards based on the tags assigned to them.
Information Displayed for Each Hazard
When viewing hazards associated with a tag, the display is structured to present key information in a clear format. The following details are shown for each hazard:
- Tag Name: The name of the selected tag.
- Assessment Name: The assessment to which the hazard belongs.
- Hazard Name: The specific name of the hazard.
- Scoring Information: Detailed scoring relevant to the hazard.
- Actions: Any related actions taken to address the hazard.
- Safety Cases: Associated safety cases linked to the hazard.
- Findings: Any findings linked to the hazard.
Detailed Hazard View
The Tag Contributors section is interactive. Users can select any row to open a more detailed view of the corresponding hazard.
- The detailed view provides in-depth information about the selected hazard, including:
- Potential Consequences: Descriptions of the possible outcomes related to the hazard.
- Mitigation Planning: Details about the strategies in place to reduce or manage the hazard.
- Risk Scoring Metrics: A comprehensive view of the risk scoring related to the hazard.
- Related Safety Cases and Findings: Information on safety cases and findings that contributed to the hazard’s creation or assessment.
